Here’s a great resource that covers the Feedback Module which Wodonga TAFE uses to collect feedback from students to ensure quality learning outcomes and experiences.  In addition the module allows you to save and reuse feedback templates within and across classes.  The feedback module is a 3rd party plug-in available for 1.9 and has been incorporated as a standard feature in Moodle 2.0′s core: http://docs.moodle.org/en/Feedback_module.  You may wish to download and install the Feedback module from the plug-in repository as well: http://moodle.org/mod/data/view.php?d=13&rid=95.
